About Academic Urology

Cole & Van Note is investigating a data breach involving Academic Urology & Urogynecology of Arizona, a division of Palo Verde Hematology and Oncology, a medical practice based in Phoenix, Arizona. [1]

On February 13, 2026, Academic Urology began reporting a cybersecurity incident involving unauthorized access to its network. According to available disclosures, a forensic investigation confirmed that between May 18 and May 22, 2025, an unauthorized party accessed files within the practice’s systems before the intrusion was discovered on May 22, 2025. The organization launched an investigation upon discovery, took steps to secure its systems, and began assessing the scope of the breach. Regulatory filings indicate that approximately 73,281 individuals were impacted, with notification letters mailed beginning on February 12–13, 2026. [2]

Further reporting indicates that the breach involved the exposure of highly sensitive personal and protected health information. Healthcare-related breaches of this nature are particularly serious due to the inclusion of both identifying and clinical data, which can significantly increase the risk of identity theft, medical fraud, and long-term privacy harm. The organization has stated that affected individuals are being offered complimentary credit monitoring and identity theft protection services through IDX, with an enrollment deadline of May 12, 2026. [3]

The information potentially affected in this breach may include:

  • Full name
  • Social Security number
  • Date of birth
  • Driver’s license or government-issued ID number
  • Financial account information
  • Credit and debit card information
  • Health insurance information
  • Medical information, including diagnoses, conditions, lab results, and medications
  • Other personally identifiable information
