About EyeCare Partners

Cole & Van Note is investigating a data breach involving EyeCare Partners, a network of ophthalmology and optometry practices based in Ballwin, Missouri. [1]

On February 12, 2026, EyeCare Partners began reporting a data breach tied to unauthorized access of ECP-managed email accounts. According to available disclosures, suspicious activity was first identified on January 28, 2025, and a forensic investigation determined that unauthorized parties accessed multiple email accounts between December 3, 2024 and January 28, 2025. These accounts may have contained sensitive personal and protected health information. The organization launched an investigation upon discovery, took steps to secure its systems, and completed a review of the impacted data in November 2025. [2]

The investigation determined that files within the compromised email accounts contained personal information belonging to approximately 17,110 individuals. While full medical records and clinical notes were reportedly not accessed, the exposure of both personally identifiable information (PII) and limited protected health information (PHI) still presents significant risks of identity theft and privacy violations. [3]

The information potentially affected in this breach may include:

  • Full name
  • Contact information
  • Social Security number
  • Date of birth
  • Driver’s license or government-issued ID number
  • Health plan information
  • Limited clinical or treatment-related information
  • Other personally identifiable information
