Cole & Van Note is investigating Insight Hospital and Medical Center, a healthcare provider headquartered in Chicago, Illinois. [1] The organization operates as a full-service medical facility providing emergency care, specialty services, and community-based healthcare programs in the Chicago area. [2] Healthcare providers routinely collect and store highly sensitive patient information, making it critical that appropriate safeguards are in place to protect medical and personal data from unauthorized access.
In February 2026, Insight began notifying affected individuals of a data breach that originally occurred on August 22, 2025. Forensic investigations indicate that attackers were able to access the hospital’s network between August 22 and September 11, 2025, suggesting a prolonged period of unauthorized access. During this time, threat actors were reportedly able to extract large volumes of data, with estimates indicating that up to 500 gigabytes of sensitive information may have been exposed. [2] [3]
Reports also indicate that multiple ransomware groups, including LockBit5 and Termite, have claimed responsibility for the attack and have publicly posted stolen data. Incidents involving ransomware and confirmed data exfiltration are particularly severe, as they suggest that sensitive information may not only have been accessed but also retained or distributed by unauthorized parties.
The types of information potentially exposed in this incident may include:
